 Centric Health - Community Advantage

Maximizing mobility is key to independence

Our skilled Occupational Therapists conduct mobility assessments to determine what equipment may be required to maximize your mobility and independence. Your equipment solution may be a cane, walker, scooter, or wheelchair and our therapist will ensure proper adjustment, fit, and training for you to ensure your safety and success.

Funding and support

The cost of medical/therapeutic equipment and architectural changes required for your home to help you be more independent, or to manage your care better, can be significant.

Our Occupational Therapist or Social Worker can assist you in determining appropriate funding sources/community agencies that can be approached to support the purchase of therapeutic equipment etc. Assistance in completing applications and providing funding documentation can also be provided.

Assistive Devices Program

The Assistive Devices Program provides funding to Ontario residents with long-term physical disabilities to obtain personalized assistive devices that enable them to function more independently.

Occupational Therapists and Physiotherapists who are ADP Authorizers can assess the individual’s needs, prescribe appropriate devices/equipment and assess for funding eligibility.  Mobility equipment such as walkers, strollers, standers, wheelchairs and wheelchair seating can be prescribed and receive funding. In addition, communication and writing aids can also be prescribed for funding.
